This could be done in different ways: I think that reporting to an official office and poking your finger with a lancing device then making a blood fingerprint on top of a traditional signature should be sufficient.  That obviously is put into a sealed plastic sheet for health reasons: (is possibly a census of blood borne disease in relation to where you got healthcare and the horrifying reality that your trusting us to know your blood type=but we are trusting you to be honest about health care coverage) if other concerns exist such as toxicological exposure are relevant or occured for other than healthcare causes a form section is available to write your symptoms and suspicions along with a consent to GC-MS an additional sample given in form of a capillary tube {similar to the prerequisites for plasma donation}

Note: 4th/18th/21st is the supreme law of the land and never ever can this data (specifically blood type and GC-MS results if opted for) be furnished to treasonous entities like the DEA for example.  Nor can it be given to the organ traders from murder like the DHHS and hospitals you didn't consent to have that information etc common sense.

  The complication is the length of time and ability to report frequently so a standard notary office like a bank or something might be appropriate but not the same place as recieving healthcare for obvious reasons.

note the only controversy is the care for terminally ill that are given life extension therapy but remain unconscious: this situation is rare as most people do regain competency before death unless given "mercy killing" treatment like the typically 13 months after admission to a nursing home planned death date overdose of morphine etc.
